Why does my Dish DVR sometimes split one episode in to one minute segments?

Once in awhile I will be shocked when a TV show reads 48 new episodes when I know there has only been one. Upon investigating I find that the DVR has split one episode in to 40 one minute segments. What would cause this to happen? Any information is appreciated. Thanks!!

    A bad dvr, get it replaced as soon as you can
